A free reading-notes worksheet for your own book, your own explanation, and one question to revisit.

Set a question before opening the chapter
Choose a question the reading might help you answer. It can be narrow: “Why did this character change their mind?” or “How does this process work?” Record the book and chapter so you can find the passage again. A question gives your notes a purpose without requiring you to summarize every paragraph.
Separate the author’s words from your explanation
After a short section, pause and explain the main idea in your own words. If you copy a brief quotation into personal notes, mark it as a quotation and record the page. Do not upload or republish a chapter or copyrighted illustration as a substitute for your own notes. Study Dimension’s account dashboard is still a developing shell; this public worksheet is a usable, independent way to prepare a reading note now.
Revisit the idea before checking the page
On another occasion, try to answer your original question from memory, then compare your answer with the book. Mark what you understood and what still needs clarification. A changed explanation is useful evidence about the reading process, not a score of your intelligence. The tool saves no reading progress to an account and has no automatic reminder. Download the note and choose your own review time.
Worked example
A small situation to try
Illustrative reading note: a reader asks why a fictional village changes its water system. After the chapter, they explain the reason in two sentences, mark one uncertainty, and note the chapter reference. The next day they try the explanation again before reopening the book and correct the detail they missed.
